Supportive Housing Case Manager

Care Corps’ LifeHouse is seeking a compassionate, organized, and motivated Supportive Housing Case Manager to join our team serving individuals and families experiencing homelessness and housing instability. This full-time position works directly with participants in our scattered-site supportive housing programs to help them achieve long-term self-sufficiency.
 
Key Responsibilities:
• Provide individualized case management and goal planning for participants in supportive housing programs
• Assist individuals and families with housing stability, employment, budgeting, benefits, education, and life skills
• Coordinate referrals for mental health, medical, substance abuse, and community resources
• Maintain accurate documentation and data entry in HMIS
• Monitor participant progress and program outcomes
• Build strong partnerships with community agencies and service providers
• Collaborate with shelter and housing staff to support successful transitions into housing

 
Qualifications:
• Associate degree or higher in human services or related field preferred
• Experience in case management, direct service, or human services preferred
• Strong communication, organization, and documentation skills
• Ability to work independently while managing multiple priorities
• Proficiency with computers, Microsoft Office, and database systems
• Valid driver’s license
• Bilingual abilities are a plus

 
Schedule:
Monday through Friday with flexibility based on program needs. Some evening, weekend, holiday, and on-call responsibilities may be required.
