ACT Peer Support Specialist
The ACT Peer Support Specialist draws from personal experiences of recovery and wellness to provide compassionate support, advocacy, and guidance to individuals served by the Assertive Community Treatment (ACT) team. This role is vital in promoting hope, self-determination, and community integration for adults with severe and persistent mental illness (SPMI). The specialist collaborates closely with the multidisciplinary ACT team to deliver comprehensive, person-centered, and trauma-informed care that empowers clients to achieve recovery and independence.
Key responsibilities include building trusting relationships with clients, supporting goal setting and self-advocacy, modeling recovery principles, and facilitating community engagement. The specialist provides on-call support, assists with transportation, documents client progress, and participates in team meetings and quality improvement initiatives.
