The ACT Nurse (RN or LPN) provides direct nursing care, medication management, and health education to adults with severe and persistent mental illness (SPMI) through the Assertive Community Treatment (ACT) program. This evidence-based model supports individuals most at risk of psychiatric crisis, hospitalization, or involvement with the criminal justice system. The nurse works as part of a multidisciplinary team to promote recovery, independence, and well-being through community-based, person-centered care.

Responsibilities include conducting health assessments, administering and monitoring medications, providing crisis intervention, and coordinating care with prescribers and healthcare providers. The nurse delivers education on wellness, medication adherence, and chronic condition management while supporting clients in achieving recovery goals. This position also involves participation in team meetings, accurate documentation, on-call rotation, and quality improvement efforts.
