ACT Therapist I-IV

The ACT Therapist I–IV provides therapeutic and clinical services as part of the Assertive Community Treatment (ACT) team, serving adults with severe and persistent mental illness (SPMI). This role focuses on evidence-based, person-centered care to promote recovery, independence, and community stability. The therapist collaborates with a multidisciplinary team—including psychiatrists, nurses, case managers, and peer specialists—to deliver comprehensive mental health treatment in both office and community settings.

Responsibilities include conducting assessments and diagnostic evaluations, developing individualized treatment plans, and providing evidence-based therapy (individual, group, and family). The ACT Therapist also participates in crisis response, medication distribution, and integrated care planning. Additional duties involve on-call rotation, clinical consultation, documentation, and participation in quality improvement initiatives.

Therapist levels (I–IV) correspond with education, licensure, and experience requirements, ranging from provisional licensure to full independent practice.
